A research team led by Bassam Jreos Faris from the University of Ottawa collected and analyzed the results of six reports covering five controlled clinical trials conducted in several countries (South Korea, the United States, Brazil, Israel, and India).

These studies included 238 participants with an average age of 36 years, most of them women, with two studies focusing only on women.

The participants varied between those suffering from Major Depressive Disorder and those with bipolar disorder in a depressive phase.

  • Two trials focused on women with Major Depressive Disorder showed additional benefits when taking creatine:
    • In the first, creatine was taken with the antidepressant escitalopram for eight weeks, and there was a significant improvement in reducing depressive symptoms (with a large effect according to the Hamilton Depression Rating Scale, Cohen’s d = 1.13), along with an increase in the number of cases that achieved full remission.
    • In the second, creatine was combined with cognitive behavioral therapy, and the participants experienced greater improvement compared with cognitive behavioral therapy alone with placebo.
  • Three other trials did not prove a clear benefit:
    • A trial showed no improvement with doses of 5 or 10 grams daily in patients with treatment-resistant depression.
    • A trial involving teenage girls did not show any noticeable effect of creatine compared with placebo, even with changes in doses.
    • A trial on patients with bipolar disorder showed no positive effect.

The researchers noted caution regarding creatine’s effects on patients with fluctuating mood conditions. In two people with bipolar disorder, mania or hypomania developed after taking creatine, indicating the need to understand its varying effects depending on the individual’s condition.

Nevertheless, the researchers stress that the relationship between creatine pathways and mental illnesses is still at the hypothesis stage, with no direct evidence proving that disorders in creatine metabolism cause depression.

The researchers recommend conducting larger studies of longer duration, exceeding eight weeks, while continuing to investigate the effects of different doses and studying the possibility of combining creatine with exercise and the effect of that on outcomes.

Current evidence does not support adopting creatine as a sole or primary treatment for depression, but it appears to be an option worthy of further research, especially with animal studies indicating differences in response between males and females.
